If you are visiting us for the first time, please download and complete these forms prior to your appointment. The forms should be viewed and completed using Adobe Reader, available below. *Google Chrome is not supported for new patient forms* If downloading the forms from Chrome, click the arrow to the right of the download, and click “always open in Adobe Reader.”
Forms can be emailed directly from the “submit” button at the bottom of the form, but Adobe must be configured to use the email account on your computer. You will get a final chance to personalize the submission before it is sent. Forms can also be submitted directly to frontdesk@growbigsmiles.com
Appointments
Appointment times are reserved exclusively for your child. We ask that you please notify our office 24 hours in advance of your scheduled appointment time if you are unable to keep your appointment.
Our doctors encourage parents of preschool children to request morning appointments, when children are fresher and more receptive to new experiences. School-age children can be seen throughout the day (dental appointments are generally an excused absence).
If your child experiences pain or has an emergency situation, please see our Emergency Procedures.
Accompanying Your Child
You are always welcome to accompany your child to the treatment room. We invite you to stay with your child during the initial examination. During future appointments, we encourage you to allow your child to accompany our staff through the dental experience as we work to develop that all-important bond of trust and rapport.
Should your child need treatment, you are also welcome to join us in the treatment area. Special chairs are provided for parent comfort. However, we ask all parents to be a “silent partner” in the treatment rooms so that Dr. Moore, at all times, engages the attention and cooperation of the patient under her care.
X-Rays
As part of providing gentle dental care for children, Pediatric Dentistry of Central Georgia utilizes the latest, low-dose digital Xray technology. X-rays, or radiographs as they are more properly called, allow us to diagnose and treat health conditions that cannot be detected during a clinical examination. If dental problems are found and treated early, dental care is more comfortable for your child — and more affordable for you. Dental X-rays represent a far smaller risk than an undetected and untreated dental problem.
